Greater Manchester’s dementia care pathway

We’ve been told that finding the right information and support when you’re living with dementia, caring for someone living with dementia or delivering services for people living with dementia can be difficult. That’s why we’ve developed this resource

The Dementia Care Pathway has been developed Dementia United. We worked with people living with dementia, their care partners, and professionals in Greater Manchester to find out what matters to them and to use this information to co-produce a set of 75 dementia quality standards.

These quality standards provide information and resources for all stages of the dementia journey, from looking after the health of your brain, receiving, and managing a diagnosis, staying connected within your local community, to what to expect from end-of-life care and much more. Each standard includes information on how to access support and services across Greater Manchester and nationally, as well as evidence as to why the standard is important and examples of best practice showing how it has been successfully implemented.

Whether you’re a member of the public looking for local support and information for yourself or a loved one, a commissioner looking for best practice guidance on how to support people in your community or work in health and social care and are looking for the evidence behind different interventions, you’ll find it in our dementia care pathway.

I believe the dementia care pathway is a wonderful initiative which will serve to enhance the lives of people living with dementia. It provides clear guidance on what people can expect when accessing support. The dementia care pathway will also be a great education and training tool for health and social care staff and ensure services are much joined up too. John is a lived experience advisor, a Dementia United governance board member and an advisor for the Commons Select Committee for Health and Social Care.

In partnership with Health Innovation Manchester we commissioned Radical to develop the digital version of the Dementia Care Pathway.
