Working to make dementia and delirium data easier to access and understand

By using Greater Manchester’s advanced data tools Dementia United and the Greater Manchester Data, Insight and Intelligence team are aiming to create clear and useful insights that improve care and outcomes for people living with dementia and delirium across the region.

We’re developing a dementia data dashboard using information from NHS England Digital. This dashboard includes key details like:

  • Dementia diagnosis rates.
  • Types of dementia.
  • Gender breakdowns.
  • Care plans.
  • Antipsychotic prescription trends.

This tool will provide a complete picture of dementia care across Greater Manchester, helping us tailor support for different groups in the community. You can access this dashboard here Primary Care Dementia Data Dashboard

We’re also developing a Delirium Emergency Admissions Dashboard that tracks emergency hospital visits, focusing on cases where delirium is identified. You can access this dashboard here Delirium Emergency Admissions Dashboard

Our priorities for 2025

  1. To fully establish a dementia data dashboard for Greater Manchester to inform good practice models & pathways, and to support quality improvement across Greater Manchester.
  2. Develop the memory assessment services dashboard.
  3. Memory assessment services play a key role in supporting people with dementia. We’re working with Pennine Care Foundation Trust to include memory assessment services data in our dashboards, improving our understanding of dementia needs and shaping care plans.
  4. Automate the dementia dashboard. We’re working to automate the dashboard to ensure it always shows the latest information. This will help professionals make faster and better informed decisions about care.
  5. Map patient pathways for delirium admissions. We’re studying how patients move through the healthcare system during and after a hospital stay for delirium, focusing on:
  • Admissions and discharges, particularly for people in care homes.
  • Annual updates on GP coded delirium data with teams in Wigan and Trafford.
  • Linking population data to hospital admission rates to understand how common delirium is.
  • Highlighting primary and secondary delirium diagnoses during hospital visits.
  • Including cases of delirium linked with dementia, using NHS data from the National Dementia Programme.
