Dementia United has developed one of the most progressive and integrated delirium care models in England, making significant strides in community-based detection, prevention and management.
The Greater Manchester community delirium toolkit, used by GPs, nurses, paramedics, urgent care teams and care home staff has been instrumental in keeping people with delirium out of hospital, ensuring they receive prompt care in familiar surroundings.
A team of clinicians from Singapore travelled to Greater Manchester for a month-long study visit in February/March 2025 to learn how our health and care services work together to improve delirium care.

Living with dementia can be really complicated for both patients and their carers. Understanding how their illness may progress or how things might impact them such as delirium is really crucial so they can be managed at home and in the comfort of their own home and with their carers around them. By recognising deterioration like delirium we can put interventions in really early to make sure it’s less frightening for both them and their families. What we’re doing in Greater Manchester is making sure people have the resources and training available to understand delirium early, access the right resources in a connected way to make sure they can support those individuals at that time.
