As part of World Delirium Awareness Day on 13th March 2024, we are pleased to announce that our fabulous delirium community and hospital toolkits have been refreshed for 2024.
Dementia United have developed several Greater Manchester delirium resources. These resources are based on national evidence and are co-produced by experts by experience.
The Greater Manchester community delirium toolkit (Updated January 2024) is designed for health and social care staff in the community and primary care to identify and manage delirium. The toolkit features a number of key documents including:
- Community process for identifying and managing delirium
- 4AT delirium screening tool
- Community delirium TIME Bundle (Triggers, Investigations, Management, Engagement)
- Community delirium management and engagement guidance.
The Greater Manchester hospital delirium toolkit (Updated January 2024) is designed is designed for hospital staff to use to help them identify and manage delirium. The toolkit features a number of key documents including:
- Hospital process for identifying and managing delirium
- 4AT delirium screening tool
- Hospital delirium TIME Bundle (Triggers, Investigations, Management, Engagement)
- Hospital delirium management and engagement guidance – for medical and non-medical teams
