Dementia Wellbeing Plan Digitisation
Our aims
The Greater Manchester dementia wellbeing plan promotes personalised planning conversations with people living with dementia and carers about their needs and wants. The plan ensures an improved standard of care planning for people living with dementia and also facilitates sharing across the system. It will be available as a standardised plan which can be accessed and shared digitally between practitioners.
Why it matters
Across Greater Manchester we know that there is great variability in how care planning is being undertaken. We know that receiving a diagnosis of dementia may have a significant and lasting psychological and emotional impact on people with dementia and their families, requiring a period of adjustment and ongoing support. Post-diagnostic support must be tailored to individual needs rather than taking a one-size fits-all approach. There is a great opportunity to improve the current situation with the implementation of a standardised Greater Manchester dementia wellbeing plan.
Wellbeing Plan or Care Plan?
Many people and resources talk about a care plan for people living with dementia rather than a wellbeing plan. They are very similar things and both cover discussions about the strengths and needs of people living with dementia and their carers. We have chosen the name Wellbeing Plan as the goal is to have a less clinical and more personal focus: the plan is about the individual’s wellbeing, their strengths and goals, and not solely on the care which will be provided.
What we have done so far
We collaborated with experts by experience to ensure that the content of the dementia wellbeing plan focused on the priorities for people living with dementia and family carers. This included attending the Dementia Carers Expert Reference Group meetings, EDUCATE in Stockport and Salford Dementia Champions Network group; where we focused on the dementia wellbeing plan. We also facilitated workshops at the Dementia Voices Network Celebratory event hosted by the Alzheimer’s Society on the 14 October 2019.
Following the co-production of the content of the plan, we then delivered training and piloted the Dementia Wellbeing plan in three localities. Please refer to the report of the training and pilot.
